Single Membership

Per Year
- One comprehensive annual exam with cleaning, full mouth x-rays, and fluoride treatment

The total retail cost of all included services for a single member is $871.
Periodontal Membership

Per Year
- Four periodontal cleanings
- One comprehensive annual exam with cleaning, full mouth x-rays, and fluoride treatment
- One six-month check-up exam with cleaning, check-up x-rays, and fluoride treatment
- Two emergency exams with necessary x-rays
- 20% off all general treatment
- 35% off scaling and root planing
- $500 off Invisalign (prices start at $4,500)
This plan is available exclusively to patients diagnosed with periodontal disease.
Couple Membership
$455
Per Year
- One comprehensive annual exam with cleaning, full mouth x-rays, and fluoride treatment
- One six-month check-up exam with cleaning, check-up x-rays, and fluoride treatment
- Two emergency exams with necessary x-rays
- 20% off all general treatment

Family Membership

This plan covers the membership of a couple and a single child. There is an additional fee of $179 for each additional child.

DAG Prime Plan
Patients who opt into a DAG Prime plan pay one upfront membership fee to gain access to all inclusions under their chosen plan. Additionally, members are entitled to 20% off any restorative work completed at our office.
Paying into DAG Prime helps to encourage frequent and regular preventative dental visits, which enables small dental problems to be addressed preventatively before they become bigger and more costly problems; this is saving YOU money in the long run and improving your dental health.
